What do we do?

If you have concerns about memory problems or dementia regarding yourself or a family member, or are a carer for someone who is experiencing memory loss, our GPs will be able to offer a memory screening test before referring you to a memory service for further assessment by the specialist Memory Service  team. Patients with dementia have a minimum of twice yearly dementia/general health checks. The surgery has copies of the Devon Carers’ Pathway Booklet, which we can supply to interested patients and their carers.   The surgery can also provide a carer’s pack, including advice on benefits and other sources of support

How to access this service:   

Patients registered at North Road West Medical Centre should book an appointment to see their GP at the surgery, if they are able to attend a surgery appointment.   If not, they or their relatives or carer can request a home visit for assessment.
